I cut 4 back like that last week for a customer. I put on welding leathers over my arms and shoulders and a full face mask. Reached in with a 36 inch pruner and used my clearing saw a bit on a few thick branches. It all sucked ass but it worked lol. I'm sure there's a better way. Every one else she called passed on it or wanted 400. Took me an hour and a half and got 225. That was her offer not my bid.

Cut it back. I've done them as big as you described, with stems so big that loppers were too small. Roses generally bloom on new wood.

Cut it back like they want it. The customer is not always right but if that is what they want, do it. I would also fertilize real good after, that way it will come back fast. It will bloom this year regardless if you fertilize or not.
