Business forms - HELP!

I am looking for any ideas on these forms.....
1. employee expectation form (for reviews, and new hires)
2. vehicle accident form (tying to capture what happened,who was involved
where it happened, all insurance info,procedures to follow afterwords, etc.)

3. personnel injury form ( same type of thing, who, what where, why,) not sure if something like this is required for workman comp. anyone know?

4. equipment damage form (again, same idea, what happened, why, what is damaged?)

Hopefully I can get some takers on this. thanks for the help!! Jeff

jsf343 - you're quite welcome. I'm glad I haven't had the chance to use these...with an accident form, there would have to be accident...which would mean they weren't careful, hah. and I use the two employee templates ONLY when someone is slacking off or something of the sort. GOOD LUCK!
