When planting a tree, how big should the hole be, and should the ball stick up above the ground at all? Do you mix something in with the soil too?

Dig a $1 hole for a $0.50 tree. In other words make it twice as wide as the root ball. (not deeper) Plant it about an inch higher than surrounding soil. The trunk flare should be above grade. Sometimes there is soil covering the flare under the burlap. Adjust planting height accordingly. Remove as much of wire basket, around root ball, as possible. It can lead to root girdling
