OSHA ruled that Farrin Dakin failed to provide a safe workplace since employees were working under and adjacent to elevated and improperly secured lifting arms.
Why you should never override safety switches.

People do some REALLY stupid things. Someone gets too comfortable with a piece of equipment, takes a shortcut and gets cut short. I saw someone use a forklift to raise up a tractor with a chain to work underneath, of course it fell and crushed him and that was that. Also saw someone drown operating a skid loader by a canal bank and later go over the edge into the water. I feel bad because when it happened I was not there to help.
It must happen fairly often as if you go to the OSHA website you can read about a lot of them. This is why we have seat sensors, arm bar sensors, push to move buttons etc. Yet people continue to try and bypass these and continue to kill themselves. Maybe we need an IQ test before you can start the machine.

did this bobcat have a seat switch? the article said he attempted to switch buckets with the safety bar down. seems he leaned over it to release the bucket, and stood on the lift peddle. unless there was a seat switch that was bypassed, sounds like operator was at fault.
