Anyone have a check out form?

I am looking for a simple form so when I give a new hire uniforms or a 2 way radio etc. that they know they are responsible for them. They must be returned in acceptable condition at the end of their employment or if they loose or destroy something while employed they will be charged to replace them.

I'm not talking about tools in day to day use but items I loan them to use and they take home they are the only ones to use them etc.

Do you do your uniforms through a service? I worked for a uniform company, and they actually provided us with "Uniform Responsibility Forms" to give to employers. They have a space for listing everything issued to the employee, replacement costs, and the employee and you sign it. Wish I had one to give you, but that was a long time ago.
