need proposal form

hello all, my proposal form is very basic, and i wanted to change it to give a more professional apperance. I was wondering if maybe a few of you would be willing to let me take a look at yours to see what i can add to mine. please help..

I'm not sure if you read my post yesterday about this, but when you get a bunch of these sample forms, proposals, contracts, etc. Upload them onto the files section of my website so they'll be there for everyone. We can all get some great ideas then.
