I did a search on here for fertilizing cost and one thread said they charge $.008 per sq. ft. I was wondering if that price included the cost for materials?

You can't base price simply on size. Lets say I have a 100 s.f. lawn, are you going to treat it for 80 cents?

Most companies have a stop charge or minimum price that covers properties up to a certain size and then add on to that minimum as the size increases. For example, for any size lawn from 1-2000 sf, I have a flat charge of $30.00. For a 3K lawn , the cost is $33.50, which is my $30.00 stop charge for lawns up to 2K plus $3.50 for the additional 1K, a 4k lawn would be $37.00 ($30 +3.50+3.50), etc, etc up to about a quarter acre. At that point the cost per 1,000 drops a bit but I'm sure you get the idea.

The idea of a stop charge is to cover all of the cost of treating a lawn before you get out of the truck plus some profit. You need to cover your gas, insurance, equipment cost, over head etc. Then the additional charge per 1,000 sf covers your materials plus your added time to treat each additional 1,000 square feet.
