Take a look at what TruGreen and some of your competitors are doing. After all the goal is a sale of your services. I only quoted the ap price and the number of applications per year.
My estimate sheet had a box to check next to each weed, disease or problem grass. For each problem its best to print a line to reassure the customer that you will spot and take care of the problem--or at least help it. I suggest put your logo and company information at the bottom. Put your phone number and "CALL NOW" at the top.

It looks like you have grouped the difficult weeds together with the controllable weeds.
Only 4 grass types--no tall fescue?
