Take a peak at my contract

I'v been trying to organize the services selection area. since you can have monthly/seasonal charges or per service charges. or a combination. I know how to fill it out because i made it. But how do other people see it.

is it understandable?

Looks good except a few things I personally would change. Change Chose to either choose or chosen. Name of credit card holder should be included. I'm a little confused about the as needed part of mowing,but then you give them the option of weekly or bi-weekly?

wife said the same thing. i just don't have room for it in this design.

Ok I don't want to sound dumb here but who fills out the prices on the right hand side of the desired per occurrence service? Is this a priceline sort of deal where the customer writes there desired price? Not being a smart a#s but I see that and I'm confused if I were your customer...

Contractor fills out the prices and homeowner/client checks the box for the service they want........not confusing

"per occurrence" refers to how its billed.

billed monthly, seasonally or billed per mow.

I agree with the others the terms/conditions should be on the back side. Also I think the services should be more structured and not all over the place.
