I worked for Wright Tree service in the past and put 12 in log / Branches and maybe bigger through that same chipper. My heart goes out to his Family. So 1 of 2 thing prob happened, Threw a a large long limb in with a bunch of shoots and it grabbed him and pulled him in with it, or had gloves on and tried to push some stuff in and it grabbed his hand. Horrible!

I've used many and only a handful had a brake bar.
It was probably quick too.

I showed this video to a couple of my guys earlier. They have on more than one occasion jammed an item behind the bar to lock it in place, hopefully this will remind them why they shouldn't do that.
