Employee Saftey gear form

Hello
I am looking for a form to give my employees at the beginning of the season when i give them there new safety Equipment. I need them to sigh this for my own protection
Hearing protection
Safety glasses
Dust Masks
Respirators and so on
if any one has one they could post or any ideas

Safety Equipment Acknowledgment Form

I acknowledge that Cape Atlantic Landscaping has provided me with the following safety gear for use while operating equipment. I understand that while operating equipment I am required to use the all safety equipment necessary for that specific machine. In addition, I understand that upon ending my employment relationship with Cape Atlantic Landscaping all safety gear is to be returned on my final day of employment.

Adjust as you see fit. Also if your state allows you to you may be able to dock their final pay check for the cost of the safety equipment if it isn't returned.
