Need tips on first Soil Test

Hey guys, I am about to take a soil sample and send it out to be tested. I have a couple questions first.

1. It is a decent size yard about 1 acre. Is it good to just say take a sample from the back yard or is a better idea to take a samples from different areas of the lawn and mix them together? (I would like only have one sample tested for cost reasons)

2. Here is info on the test through MSU extension program does it look good? http://www.oakgov.com/msu/assets/doc.../2011_dgst.pdf
