Piston Slapper, any one of the answers could be the cause of the condition you described especially if the tech working on the engine had no idea what he was doing.

Very close and your concept is correct. But there's a deadlier combo where the error is actually intuitive.

It's not a joke and it does happen a lot to a specific group of people.
