Contract Labor Contract??

I have in the past and I am planning on this year hiring a lot of contract labor....friends and family mostly. I do a lot of sod jobs, and contract labor works perfect for that. Is there anyone that has a contract that they give to their contract labor. Stating things such as...

We are not responsible for personal injury.
If you do more than $600 a year, expect a 1099 misc.
You are your own contractor, please bring your own tools.

Basically letting them know their status and responsibilities?

Does anybody have anything out of the norm on their contract? Such as stating the rules for being an independant contractor?

For anyone interested, I also found a great IRS form W-9 for requesting SS numbers or EIN numbers and addresses. Just search for W-9 on IRS.gov
