Sub Contractor release forms

Does anyone have a form they make your subcontractors sign to say they are providing their own workers comp? Could you post it or e-mail.

Its way better to get a certificate of insurance from their agent. No confusion about whether the insurance is still in force.

Lanelle is right, and this is common practice too.
However, don't let them send you the Certificate, make sure it comes from the agent (fax copy is acceptable).
