WTF kind of PVB installation is that? Faucet in between the shut-offs?.... for that matter, where is the second shut-off?

Not to CA code, but still meeting ASSE 1020 requirements for a PVB. Remember that these things work on gravity, so if the elevation is right, it's job done. All essential tests for the functioning of the PVB's check valve can be done by shutting off the upstream valve.
