Soil Test Help!

I just recently received the results from the soil test that was done by Scotts. The guy was pretty much usless in his explanation; he actually stated that he "uses fertilizer, but doesn't know how to read the results". I am looking for some recommendations based of the following results. Thanks in advance! Tom
