Forms you leave with customers

Does anyone have a copy of the form/letter they leave with customers after doing an application? Searched and couldn't find anything. Have made my own, I am just curious if I left out anything, and to see how other format theirs.

We normally leave a statement at the time we fertilize. If it is a client that will get other work done that month (as we normally bill monthly) we leave a card on the door saying we were on the yard and that a statement will follow.

Your not telling basics of what was applied, rate, type etc? Mine was formatted to easily transfer info for state requirements but am afraid I'm giving customers to much info.

Put the important stuff like "Pay this amount" up top. Many times they will not read too far down the page.
It is convenient to put the government required data on the same sheet. Give cus the top copy and the carbon goes back to the office. In my state gov required name, number, concentration, lbs per acre of the product, target and equipment type.
