The piston experienced a lack of lubrication...
Possible bad fuelmix...straight gas...major air leak..heavy dirt ingestion... The sparkplug on the right was hit by the piston.... I would have thought it was the wrong sparkplug (threads too long) but judging by the dirt buildup on the porcelain...it was installed and working for some time..

Ever have a Kohler muffler that rattles when you remove it.???
It used to was Kohler's official stance that those little broken pieces of the baffles could NOT re enter the cylinder via the exhaust valve....and damage the cylinder..piston...valves...or the head... They didn't want to warranty the muffler during the headgasket replacements that were so common... This picture is the SMOKING GUN....I got them... The intake valve guide broke in pieces on cylinder #1. And if you'll notice... There is a piece of that valveguide stuck under the exhaust valve on cylinder #2.. Cross contamination through the muffler...it does happen...

Naw. Those pics were taken with my 7 megpixel camera..
If you view them on a droid or iphone you can enlarge them pretty good... The pics ought to make you think..... the next time you bolt on a rattling muffler...
