belt problems

My brother in law has a 03' dixon ztr 4500 series and the belt keeps coming off. I told him to locate the belt tensioner but it has a spring loaded automatic one. After attaching it and mowing for a brief time before it would come off and putting it back on, now the belt won't spin at all. Not sure what to think maybe bearings or something need to be repacked? Any thought would be great.
