Application Record / Forms

    Been a commercial applicator for about two years. The records are a pain I'm trying to stream line mine. I'm from the state of Ohio, and they have no examples. They have the requirements for the form, and you design your own. I'm looking for examples with check boxes or formats that are less time consuming to fill out.
