    Try and raise the side navigation up. I'm forced to scroll down to see what you have there. Your contact page should only be for new inquires. As a potential customer I may be scared away by seeing a cancellation option before even signing up for your service. You can have a separate form for existing customers. Try using two different pages or have two links to the different forms on that page.

    Thanks... I noticed that when I veiwed it in another resolution as you must have yours smaller than mine.
