What form are you trying to fill out? Is it the BAS-1?

D.OT. Stands for Department of transportation. You need a Dot # for or when your vehicleweighs over a certain limit.

Call the phone number and have a live person walk through the sign up form with you on the phone.

I found that to be the easiest way to be sure that their definitions and mine where the same.
