Forms for the business- Help! PLEASE!!

    I am looking for ideas from ANYONE on forms your business uses for the following.......

    1. Wreck (vehicle) forms something to capture all the info immediately after a wreck. (names,location, what happened, proceedures to follow, etc.)

    2. Personal injury form - same type of thing, looking to find a way to capture what happened, where, why, etc. in a simple, quick and efficiant way
    Side question for this... is this required for workmans comp? anyone know??

    3. Safety guidlines you may have layed out for your employees.

    me too. i put a call in to my insurance guy a few days ago to see if my work comp company has anything. still have not heard. but if/when i do, i'll attach it.
