Need a proper invoice...

    I am trying to come up with a proper invoice that will be useable but not have to have the info entered for each treatment.

    Basically, what I am wanting is a form that I can have for each job, the top portion will have the information for the techy to apply the chems; property size, customer info, notes, etc etc. then the bottom half will have the invoice for the customer, tear off preferably. If anyone has any ideas other than the multi thousand dollar programs I would greatly appreciate it.
