New spray invoice for OKlahoma

    Need help for the requirements of spraying invoices in Oklahoma to update my current invoices. I have the form from the Dept of Ag, but it seems vague. I downloaded the Pesticide Record keeping Form form for OK and I have most of the info on my forms. Can't find any picture examples of spraying invoices. Suggestions or pictures of current invoices being used???? Or websites for idea.
