Quality scale survey

    I will be mailing out a survey along with my invoices this month and was wanting to know what you all would think the top five or ten questions i should let my customers grade me on? Quality of work, appearance, so on and so on. I figure this could be a good way to find out how i can better my buisiness.

    I think it's a great idea, and I was planning to do the same with my final invoices for the year.
    I thought about a lot of possible items, but was thinking it may be better to just write a letter asking them what I could do to improve service next year and provide a space for them to write their thoughts. Haven't decided yet, though.
    Some possible things:
    promptness (like returning phone calls, etc.)
    what other services we could offer to fit their needs
    ask them to give an overall rating on a scale of 1-10
    those are some of mine, hopefully someone else will chime in too, as this could be a great way to see what needs improving/changing.
