service acknowledgement

    Does anyone have to leave a slip/sheet or something whenever you do commercial accounts that you don't bill dirrectly. I have one that wants me to leave something with the store managers prior to us leaving. I have never ran into this before so I needed to ask. Also if you have a copy of something like this could you shoot me a copy or something to look at to get some ideas.

    I will attach the copy I have made. Let me know what you think.

    I assume you are doing this with a contract, I would consider adding something in the contract that states complaints or constructive criticism should be noted on the acknowledgements? Just a suggestion.
