Where do I get a credit card agreement?

    I did a search but didn't really come up with anything.

    I am using quickbooks for billing and also processing credit cards. I need to send a letter to my customers allowing them fill out there credit card information as well as signing an agreement for me to bill their card but I can't seem to find one.

    I thought that quickbooks had a template but after a call to them they said they didn't know of it.

    I knew there was one there. I thought I had seen it. Quickbooks was trying to transfer me to merchant services for the second time and they said they didn't know where it was either on the first transfer.

    if you have merchant services and use qb2007, go to customers, then credit card processing activities,then automatic credit card billing. it will take you to merchant soulutions website and has an authorization form filled out with your info. I printed one, but its my info on the form.
