did you authorize your name with the state? its something you should look into. in PA fictitious names need to be authorized but if you use your last name your fine.

I am pretty sure that in Indiana, if you are a sole-proprieter that you do not have to register your name with the state. When you fill things out, like government paperwork, you just report your company name as your D.B.A. and then yout personal name, etc. In our state, as a sole-proprieter all you must do is report everything as far as taxes through your SS#.
