I had an attorney draw up a contract for me a few years ago. The problem was it ended up being 5 pages long before you filled in the blanks. I still use it on large comercial jobs, but it seemed to much for everyday use. I condesed it keeping most of the same wording as in the original. Here is what I use for most customers, just fill in the blanks.

    Hope this helps

I have designed mine from a template out of an old textbook of mine, clients respond really well to it. Use it well =)
