Does anyone have one that they can post...

I started one but I'm sure I missed a thing or two and would like to see someone's that has more experance than myself.

Agreement for lawn service

This agreement entered into between Clean Cut Lawn Care and __________________ commencing on ________________ and continuing thereafter as long as both parties are satisfied. Each agreeing to give either party a thirty-day notice of intention to change or terminate contract.
Clean Cut Lawn Care agrees to mow, edge, trim, and blow off concrete surfaces at ____________________________________________ for a monthly fee of _________.
First payment due on ____________. Subsequent payments due on every _______ of the month thereafter until services are terminated by either party.

Yearly Contract ______ Month to Month ______
(On yearly contracts monthly payments are figured on a per cut formula and divided into 12 even monthly payments based on following service schedule. (4 free fertilizer treatments per year))
Service schedule is as follows: March – October Weekly service (35 cuts)
November – February Twice a Month (8 cuts)

(Edging and trimming will be done as needed at time of service to maintain a clean looking lawn)
Any extra services will be agreed upon in advance between customer and Clean Cut Lawn Care on a per job basis.

If you are not satisfied with the level of service you are being provided please notify Clean Cut Lawn Care immediately so that we can correct this in a timely manner. Clean Cut Lawn Care strives to complete work in a respectable manner each and every visit. 100% Satisfaction Guaranteed.
If a dog is kept on the property, customer must have lawn free of pet droppings, when contractor is scheduled to service lawn, or customer will be charged a $5 equipment clean-up fee per visit.
All lawn clippings will be disposed of in customer’s green waste bin.
Clean Cut Lawn Care is not responsible for any damage of items left on turf areas and/or malfunctioning sprinkler heads.
