This tree is a bit of an enigma. The bark is white yet it is not a birch or a beech. The leaves look like Oak but they are wide at the base and get narrow while the Oak is the opposite. Tree not doing to well..but still looking to identify it.

With that bark and those lenticels, it screams Birch to me. But not with that foliage. A hybrid of some type? An enigma indeed...

Any catkins or other fruiting structures present? That would help immensely.

I took a picture of the leaf with my phone which has the PlantSnap app. It said London Plane Tree. It has never been correct yet so I don't have lot of hope.

Nope, That's an epic fail, as a London Plane Tree is a Sycamore!

As for the OP's tree in question. I would say that it is a Variety of a ornamental CUTLEAF White Birch.

You know your trees. I searched off and on since this post and couldn't figure it out, made me curious. I think your answer is the winner.

Now all I have to do now is figure out how to get it to grow some more leaves as it is looking sparse.
