I had a patio put in and the cement truck left big ruts in my lawn. Is it possible to fill in these ruts over the existing grass with top soil and then just plant or should i tear out all the grass first including the lawn in between the ruts that wasn't damaged?

Depends on how bad ruts are,,, i had a problem like this once,,, i had the cust. run sprinkler system like 5 hrs or so a day for three days. Then i rented a mini asphalt rolller, took like 45 mins to fix the ruts,,, grass stayed intact came back nicely.

If the grass is strong and healthy, split each rut with an edging spade right down the middle,
slice it about every four feet perpendicular in both directions,
under cut it like a piece of sod,
flip them open like barn doors,
fill in with top soil,
grade it with a rake,
close the "doors",
water the hell out of it,
tamp it (with your feet or a hand tamp) to even it out.
